How do I provide proof of a name change?

An official document will need to be sent to ACHS.

Students can provide ACHS with a certified copy of a court order, a marriage certificate, or a dissolution decree that includes a name change by email to registrar@achs.edu

or by mail to:

State or Federal identification will be requested to update student record.
